2009 LPCA Membership Application

Payment Options

This year you have three different options for paying your membership dues:

1. Go to  http://www.lakepanamoka.net and click on “Join the LPCA”.  You can pay the full dues or just make a payment toward the full amount.
2. Mail your completed membership application with your payment (check or money order) for the corresponding membership and lot fees to the L.P.C.A at P.O. Box 242, Ridge, NY 11961.  (Please see the payment plan option below.)
3. Come early to a General Meeting or a major event and sign up with a Board Member.  Please bring your photos with you to obtain beach passes at that time.

Membership Levels

Please check one:

£  Full Membership Dues: $250.00

This level of membership entitles you and resident family members the use of lake beaches and waterfront activities, participation in all programs and events, discounted admission rates, all membership publications, full voting privileges and the right to hold office.

Beach Passes - Please also enclose current passport size facial photos of your family members with the ages of the children on the back of the photo so your beach passes can be mailed back to you.

New Payment Plan Option Available

1^st Payment of $100.00 due in March

$50.00 payments due in April, May and June

(Beach passes will be issued upon receipt of the final payment.)

£  LPCA Membership Dues: $200.00

This level of membership entitles you and resident family members to participation in select programs and events, discounted admission rates, all membership publications, full voting rights and the right to hold office.

***Please note that all lot fees must be paid in full in order to process this application. For your convenience, we have enclosed a statement of your lot fees.  You may include your lot fees with your membership dues check.
